Monthly Meeting Registration
PLEASE READ MEETING REGISTRATION INSTRUCTIONS AND INFORMATION BELOW: To register for the Pain Society of Oregon's Monthly Meeting fill in the registration information below. Items with an asterix (*) MUST be filled out. When you have completed your registration on this page, please click "Add to Cart". You will then submit your payment information. Payment can be made by check or online through paypal. All major credit cards are accepted. You do not need to have a paypal account to pay online. Registration includes attendance at meeting, including 1.5 CME/CEU hours and dinner. We are not able to refund monthly meeting prepayments due to attendee cancellation.
